Tuberculosis is a major global public health challenge, and a majority of countries have adopted a version of the global strategy to fight Tuberculosis, Directly Observed Treatment, Short Course (DOTS). Drawing on results from research in Ethiopia and Norway, the aim of this paper is to highlight and discuss ethical aspects of the practice of Directly Observed Treatment (DOT) in a cross-cultural perspective.

We investigate claims about the frequency of "know" made by philosophers. Our investigation has several overlapping aims. First, we aim to show what is required to confirm or disconfirm philosophers’ claims about the comparative frequency of different uses of philosophically interesting expressions. Second, we aim to show how using linguistic corpora as tools for investigating meaning is a productive methodology, in the sense that it yields discoveries about the use of language that philosophers would have overlooked if they remained in (...) their "armchairs of an afternoon", to use J.L. Austin’s phrase. Third, we discuss facts about the meaning of "know" that so far have been ignored in philosophy, with the aim of reorienting discussions of the relevance of ordinary language for philosophical theorizing. There is a standard story told about the rise and fall of ordinary language philosophy: it was a widespread, if not dominant, approach to philosophy in Great Britain in the aftermath of World War II up until the early 1960s, but with the development of systematic approaches to the study of language—formal semantic theories on one hand and Gricean pragmatics on the other—ordinary language philosophy more or less disappeared. In this paper we present quantitative evidence to evaluate the standard story (...) of the rise and fall of ordinary language philosophy, building on the topic model of over 30,000 philosophy articles in Weatherson (2022). Using a combination of qualitative judgment and a topic-model-based measurement of similarity between individual articles, we find evidence that supports the first part of the standard story, according to which ordinary language philosophy arises in the 1940s, peaks between the early 1950s and the late 1960s, and then rapidly declines. But we argue that there is also evidence of a "new wave" of ordinary language philosophy in the early 21st century that defies the second part of the standard story. (shrink)

ABSTRACT Objective: To describe how local research ethics committees (LRECs) consider and apply research ethics in the evaluation of biomedical research proposals. Design: A qualitative study was conducted using purposeful sampling, focus groups and a grounded theory approach to generate data and to analyse the work of the LRECs. Setting and participants: 11 LRECs of the Mexican Institute of Social Security (IMSS). Results: LRECs considered ethics to be implicit in all types of research, but that ethics reviews were only necessary (...) for projects that included the direct participation of human beings. The LRECs appeared to understand the importance of consent, as in the completion of a consent form, but did not emphasise the importance of the process of acquiring ‘informed’ consent. The committees considered their main roles or functions to be: (a) to improve the methodological quality of research and to verify – if applicable – the ethical aspects; (b) to encourage personnel to undergo research training; (c) to follow‐up research to oversee the adherence to norms and compliance with a specified research timetable. Conclusions: This study provides a valuable insight into how these LRECs understand the ethical review process. The emphasis of the committees was on rules, regulations, improving research methodology and research training, rather than a focus on efforts to protect the rights and well being of research subjects. The results encourage further normative and descriptive lines of investigation concerning education and the development of LRECs. (shrink)

It is a great privilege to have been invited to speak on this occasion. I shall not talk about Janet Semple's life, since I did not know Janet Semple well. I first came across her work when the Oxford University Press sent me a copy of her Ph.D. thesis, asking for my opinion as to its publishability. I groaned—yet another study of the panopticon! I opened it, started reading—and read it straight through. It was so clear, insightful, powerful in its (...) interpretation and beautifully written. I am delighted to see that it has appeared as Bentham's Prison. I was, at a later stage, equally pleased to be involved in persuading Janet to publish some of her research on Bentham's medical and health interests. I shall not try to evaluate her work today. I think I might best honour her by trying to reflect upon some of the wider issues that surround Bentham's concerns with prisons and hospitals—issues raised by her work. (shrink)

Le prolongement à la dimension 2 utilise des compositions de carrés dans deux directions et la richesse de l'algèbre qui en résulte est montrée par certains calculs de dimension 2. La difficulté de la transition de la dimension 1 à la dimension 2 est également illustrée par la comparaison de la notion de carré commutatif à celle de cube commutatif - le traitement de cette dernière nécessitant de nouvelles notions. This article will address the overall responsibilities and current projects relating to human subject protections.OPRR implements the Department of Health and Human Services' (HHS) regulations for the protection of human subjects (45 (...) CFR 46 (1991)). These regulations fulfill the responsibilities set forth in the Public Health Service (PHS) Act with regard to institutional review boards (IRB) and other protections for human subjects involved in biomedical and behavioral research conducted or supported by HHS. The PHS Act also mandates a program to clarify ethical issues in biomedical research involving human subjects and to address violations of the rights of human subjects of research (P.L. 99-158 (1985)).OPRR staff negotiate assurances of compliance with institutions conducting Federally-supported research involving human subjects. The assurances describe the policies and procedures each institution will follow to fulfill the requirements of the regulations, including specific IRB composition and functions.In addition, OPRR cosponsors with the Food and Drug Administration and awardee institutions a series of regional workshops that address human subject protections issues. Larry Shiner has risen to an impassioned defence against my criticisms of an iconic figure, claiming that I have ‘misrepresent[ed] Kristeller's central aim’ and therefore missed ‘the real shortcomings of Kristeller's essay’ and ‘obscure[d] substantive issues behind simplistic dichotomies’. These, and a series of disagreements over countless small details, take up the first part of his reply. He then proceeds to summarize his own book's achievements in correcting Kristeller's shortcomings. Shiner acknowledges difficulties in Kristeller's formulations, but accepts their purport and (...) actually expands the reach of Kristeller's thesis. Whatever else one might wish to say about these charges (which are quite impressive as a list but each disputable taken in turn), Shiner remains an unrepentant exponent of Kristeller's views. I realize that dogmas die hard, and it is only to be expected that if one challenges the central tenets of a legacy one will meet with entrenched reactions. Given the word-limit I have been allotted for my counter-reply, I will confine myself to the more substantive issues as these concern Kristeller's arguments, reserving for a possible future occasion any difficulties I may have with Shiner's own theses. My response will take the form of a series of questions followed by my own proposed answers. (shrink)

L'article fait une application précise de cette image au phénomène contemporain d'un changement culturel rapide dans le monde et dans l'Eglise. L'article soutient que l'image biblique des brebis et du pasteur ne doit pas être abandonnée comme un pûr (...) artifice culturel qui reflète une structure sociale primitive désormais entièrement désuète. Plutôt, cette image est une affirmation vitale et importante, tant anthropologique que ecclésiologique, sur la nature humaine, le besoin humain de communauté, d'autorité, de direction et d'attention. L'A. en profite pour soulever le problème de l'ordination des femmes au ministère presbytéral, et pour résoudre exhaustivement ce problème avec la substance de l'Encyclique.

Lexical Semantics is about the meaning of words. Although obviously a central concern of linguistics, the semantic behaviour of words has been unduly neglected in the current literature, which has tended to emphasize sentential semantics and its relation to formal systems of logic. In this textbook D. A. Cruse establishes in a principled and disciplined way the descriptive and generalizable facts about lexical relations that any formal theory of semantics will have to encompass. Among the topics covered in depth are (...) idiomaticity, lexical ambiguity, synonymy, hierarchical relations such as hyponymy and meronymy, and various types of oppositeness. Syntagmatic relations are also treated in some detail. The discussions are richly illustrated by examples drawn almost entirely from English. Although a familiarity with traditional grammar is assumed, readers with no technical linguistic background will find the exposition always accessible. Western Buddhist travel narratives are autobiographical accounts of a journey to a Buddhist culture. Dozens of such narratives have since the 1970s describe treks in Tibet, periods of residence in a Zen monastery, pilgrimages to Buddhist sites and teachers, and other Asian odysseys. The best known of these works is Peter Matthiessen's The Snow Leopard; further reflections emerge from thirty writers including John Blofeld, Jan Van de Wetering, Thomas Merton, Oliver Statler, Robert Thurman, Gretel Ehrlich, and Bill Porter. The (...) Buddhist concept of 'no-self' helps these authors interpret certain pivotal experiences of 'unselfing' and is also a catalyst that provokes and enables such events. The writers' spiritual memoirs describe how their journeys brought about a new understanding of Buddhist enlightenment and so transformed their lives. Showing how travel can elicit self-transformation, this book is a compelling exploration of the journeys and religious changes of both individuals and Buddhism itself. (shrink)

Background: Clinical genomic implementation studies pose challenges for informed consent. Consent forms often include complex language and concepts, which can be a barrier to diverse enrollment, and these studies often blur traditional research-clinical boundaries. There is a move toward self-directed, web-based research enrollment, but more evidence is needed about how these enrollment approaches work in practice. In this study, we developed and evaluated a literacy-focused, web-based consent approach to support enrollment of diverse participants in an ongoing clinical genomic implementation study. (...) Methods: As part of the Cancer Health Assessments Reaching Many (CHARM) study, we developed a web-based consent approach that featured plain language, multimedia, and separate descriptions of clinical care and research activities. CHARM offered clinical exome sequencing to individuals at high risk of hereditary cancer. We interviewed CHARM participants about their reactions to the consent approach. We audio recorded, transcribed, and coded interviews using a deductively and inductively derived codebook. We reviewed coded excerpts as a team to identify overarching themes. Results: We conducted 32 interviews, including 12 (38%) in Spanish. Most (69%) enrolled without assistance from study staff, usually on a mobile phone. Those who completed enrollment in one day spent an average of 12 minutes on the consent portion. Interviewees found the information simple to read but comprehensive, were neutral to positive about the multimedia support, and identified increased access to testing in the study as the key difference from clinical care. Conclusions: This study showed that interviewees found our literacy-focused, web-based consent approach acceptable; did not distinguish the consent materials from other online study processes; and valued getting access to testing in the study. Overall, conducting empirical bioethics research in an ongoing clinical trial was useful to demonstrate the acceptability of our novel consent approach but posed practical challenges. (shrink)

Elles font état d’une sagesse qui blâme l’excès et invite à la modération. Les échos de ces propos résonnent dans différentes périodes de l’Antiquité et chez des auteurs appartenant aux traditions les plus variées, parmi lesquels se trouve notamment Démocrite. Nombreux sont en effet les fragments qui attestent de sa condamnation des excès, de (...) sa réflexion critique sur les richesses et de son éloge de la modération et de l’équilibre entendus comme conditions du bien-être et de l’action bonne. « La juste proportion est belle en toutes choses » (DK 68 B 102), proclame le philosophe. Cette juste proportion est érigée en condition pour atteindre la « bonne humeur » ( euthymiê ) et le « bien-être » ( euestô ). La réflexion de Démocrite se fonde sur une sorte de principe économique qui s’exprime par la notion de « mesure » ( metriotês ). La capacité à être modéré est aussi le signe distinctif de l’être humain bien accompli (DK 68 B 70), et s’oppose aux différentes formes de l’avidité. Lorsque l’on examine les fragments qui traitent de ce sujet, l’on constate que le philosophe a dressé un inventaire qui prend en compte les domaines les plus divers de la vie. Il est en effet question de la mesure aussi bien dans le cadre des affections comme le plaisir et le désir, que dans celui de la satisfaction de nos besoins élémentaires. Il en est également question pour ce qui est de notre attitude à l’égard des biens matériels et des richesses. Dans cet article nous avons choisi de faire porter l’attention sur la réflexion du philosophe sur les richesses et les biens matériels, en prenant tout particulièrement en compte sa mise en cause du désir dans l’économie de la vie humaine. In debates about criteria for human death, several camps have emerged, the main two focusing on either loss of the "organism as a whole" (the mainstream view) or loss of consciousness or "personhood." Controversies also rage over the proper definition of "irreversible" in criteria for death. The situation is reminiscent of the proverbial blind men palpating an elephant; each describes the creature according to the part he can touch. Similarly, each camp grasps some aspect of the complex reality of death. (...) The personhood camp, in contrast to the mainstream "organism" camp, recognizes that a human organism can still be a biological living whole even without brain function. The mainstream camp, in contrast to the personhood camp, recognizes that a person can be permanently, even irreversibly unconscious, and still be a living person so long as his/her body is alive. The author proposes that hylomorphic dualism incorporates both these key insights. But to complete the picture of the entire "death elephant," a fundamental paradigm shift is needed to make sense of other seemingly conflicting insights. The author proposes a "semantic bisection" of the concept of death, analogous to the traditional distinction at the beginning of life between "conception" and "birth." To avoid the semantic baggage associated with the term "death," the two new death-related concepts are referred to as "passing away" (or "deceased") and "deanimation," corresponding, respectively, to sociolegal ceasing-to-be (mirror image of birth) and ontological/theological ceasing-to-be of the bodily organism (mirror image of conception). Regarding criteria, the distinguishing feature is whether the cessation of function is permanent (passing away) or irreversible (deanimation). If the "dead donor rule" were renamed the "deceased donor rule" (both acronyms felicitously being "DDR"), the ethics of organ transplantation from non–heart-beating donors could, in principle, be validly governed by the DDR, even though the donors are not yet ontologically "deanimated." Thus, the paradigm shift satisfies both those who insist on maintaining the DDR and those who claim that it has all along been receiving only lip service and should be explicitly loosened to include those who are "as good as dead." Even so, a number of practical caveats remain to be worked out for non–heart-beating protocols. (shrink)

In some languages every statement must contain a specification of the type of evidence on which it is based: for example, whether the speaker saw it, or heard it, or inferred it from indirect evidence, or learnt it from someone else. This grammatical reference to information source is called 'evidentiality', and is one of the least described grammatical categories. Evidentiality systems differ in how complex they are: some distinguish just two terms (eyewitness and noneyewitness, or reported and everything else), while (...) others have six or even more terms. Evidentiality is a category in its own right, and not a subcategory of epistemic or some other modality, nor of tense-aspect. Every language has some way of referring to the source of information, but not every language has grammatical evidentiality. In English expressions such as I guess, they say, I hear that, the alleged are not obligatory and do not constitute a grammatical system. Similar expressions in other languages may provide historical sources for evidentials. True evidentials, by contrast, form a grammatical system. In the North Arawak language Tariana an expression such as "the dog bit the man" must be augmented by a grammatical suffix indicating whether the event was seen, or heard, or assumed, or reported. This book provides the first exhaustive cross-linguistic typological study of how languages deal with the marking of information source. Examples are drawn from over 500 languages from all over the world, several of them based on the author's original fieldwork. Professor Aikhenvald also considers the role evidentiality plays in human cognition, and the ways in which evidentiality influences human perception of the world.. This is an important book on an intriguing subject. It will interest anthropologists, cognitive psychologists and philosophers, as well as linguists. (shrink)

Comme le disent le titre et le sous-titre de l'article, l'auteur centre son attention sur la «proposition» faite par l'encyclique Ut Unum Sint aux responsables d'Eglises et aux théologiens, d'approfondir, en dialogue avec le successeur de Pierre, la distinction entre la «substance» du ministère de Pierre et les «formes d'exercice» qu'il pourrait prendre en fonction de la «nouvelle situation». L'article ne se propose pas de faire un commentaire, encore moins d'analyser en détails, le contenu doctrinal de l'encyclique; il se contente (...) d'examiner la «proposition papale» dans le contexte immédiat de l'encyclique.
